PURPOSE
The aim of this study was to develop evidence-based nursing practice guidelines for cold application in adult care.
METHODS
The study procedure was based on the Scottish Intercollegiate Guidelines Network and Korean Hospital Nurses Association's guideline development methods. The evidence review procedures included targeted searches from databases including the Cochrane Library, PubMed, CINAHL, Embase, KoreaMed, KERIS and NDSL. The initial search yielded 6,426 articles published from 2006 to 2016. Based on the screening process, 147 articles were selected and systematically reviewed. From evidence summaries of 20 clinical questions, recommendations were formulated and graded. Draft recommendation were evaluated through external review.
RESULTS
The guidelines consisted of 10 domains and 27 recommendations. The content validity index of the recommendations was above .80. Major recommendations include 1) applying external cooling to reduce acute pain, edema, and hemorrhage, 2) selecting cooling instruments based on the purpose, patient's condition, and preference, 3) preparing and maintaining cooling instruments at an adequate temperature, and 4) intermittent cold application for appropriate durations with sufficient recovery time.
CONCLUSION
These guidelines can be utilized in both clinical and community settings to help narrow the gaps between nursing practices.

냉 요법이란 간호중재분류(Nursing Intervention Classification, NIC)의 정의에 따르면 통증, 근육수축 및 염증을 감소시키기 위해 냉을 사용하여 피부와 조직을 자극하는 것을 말한다[1]. 냉 요법 중에서 국소적 표면 냉 요법은 발열 환자나 수술 환자, 스포츠 손상 환자에게 흔히 사용되고 있다. 냉 요법은 발열과 통증, 부종, 출혈 감소에 효과가 있다고 알려져 있으며, 비 약물 중재로서 냉 요법은 임상에서 뿐만 아니라 일반인들에게도 자주 사용되는 건강관리방법 중 하나이다. 냉을 적용하면 신경전도속도가 감소하고, 통증의 역치와 적응력이 증가하며 통증을 줄여준다. 또한 모세혈관의 혈류와 근육의 대사활동이 낮아지고, 혈관은 수축되어 부종을 완화하고 출혈을 줄이는 데 도움이 된다[2]. 선행연구들은 냉 요법이 심장수술[3], 복부수술[4], 분만여성[5] 등에서의 급성 통증 감소와 안면수술[6], 안과수술[7], 개두술[8] 등에서의 부종 감소, 슬관절 수술[9]과 안과수술[7], 경피적 심장 중재술[10]에서의 출혈 감소, 발열 환자의 체온 감소에 도움이 된다는 결과[11, 12]를 보고하였다.
위의 대상자에게 주로 사용된 냉 요법 기구는 얼음 팩, 냉각젤 팩, 냉각수 순환패드, 냉각 공기 순환장치, 얼음조각 등 매우 다양하며, 냉을 적용한 방법도 연구마다 많은 차이가 있어 실무에서 직접 적용하는 것이 쉽지 않은 실정이다. 이러한 사실은 국외 선행연구[13]에서 이미 지적된 바 있으며, 국내에서도 냉 요법 적용시간이 2분부터 5시간까지, 적용간격은 0분부터 5시간까지 실무 격차가 매우 큰 것에 대해 보고하였다[14]. 냉 요법을 장시간 적용하면 허혈을 초래하는 혈관 수축이 발생할 수 있고, 장기화된 혈관 수축으로 영향 받은 조직에 혈류가 재공급될 때 재관류 손상(reperfusion injury)을 유발할 수 있다. 또한 너무 낮은 온도 등 부적절한 방법으로 적용하면 조직 관류의 과도한 저하와 산소결핍으로 인한 조직손상을 초래하거나 과도한 체온 감소, 오한, “hunting response”와 같은 반응성 혈관확장이 발생할 수 있다[15]. 이 뿐만 아니라 부적절한 냉 요법은 통증 경감, 부종 완화, 출혈 감소 등과 같은 중재의 목표에 도달하지 못할 수도 있다. 따라서 이와 같은 부작용을 최소화하고 기대효과를 극대화 할 수 있는 안전하고도 효율적인 방법으로 냉 요법을 적용해야 할 것이다. 이를 위해서는 냉 요법의 목적, 적용대상, 냉 요법 기구, 준비방법, 적용시간, 적용간격, 적용 부위, 적용방법, 주의 사항 및 부작용 등이 포함된 간호실무지침이 필요하다. 최근 전국 의료기관의 냉 요법 실무지침 및 실무현황을 조사한 연구에 따르면 전국 124개 의료기관 중 약 40%가 냉 요법 실무지침을 사용하고 있으나, 냉 요법 사용으로인한 부작용이나 냉동 젤 팩과 아이스 팩 등 냉 요법 기구에 대한 내용을 구체적으로 다룬 지침은 전체 기관의 20%수준이었다고 보고하였다. 또한 냉 요법을 발열 환자에게 적용한 경우가 62%, 수술 환자에게 적용한 경우가 21%였고 냉 요법 적용부위는 액와 62%, 후두부 14%이며 냉 요법을 위한 기구는 냉동 젤 팩 24%, 얼음 팩 23%, 얼린 수액백 19%, 비닐 수액백 4% 등으로 다양하게 사용하고 있다고 하였다. 냉 요법을 적용함으로써 29%가 체온감소효과, 24%가 부종감소효과, 21%가 통증조절효과를 기대하였으며 냉 요법을 독립적 간호실무라고 인식하는 간호사는 60%정도였고 대다수의 간호사가 실무의 격차를 줄이고 구체적 안내를 제공할 수 있는 전국 단위의 냉 요법 실무지침 개발이 필요하다고 하였다[14]. 냉 요법은 여러 가지 간호업무 중에서 실무지침이 필요한 분야로 보고되었으나[16], 최신의 근거에 기반한 성인 환자를 위한 냉 요법 간호실무지침은 국내외에서 찾아보기 어려운 실정이다. 이에 냉 요법관련 선행연구결과를 토대로 성인 환자를 위한 한국형 근거기반 냉 요법 간호실무지침을 개발하여 실무의 개선을 도모하고자 한다.
본 연구는 18세 이상의 성인 환자에서 통증, 부종, 출혈, 발열감소를 위해 냉 요법의 최선의 실무 적용을 위한 최근 근거 기반 권고안으로 구성된 냉 요법 간호실무지침을 개발하는 것을 목적으로 한다.
본 연구에서 냉 요법 간호실무지침(Evidence-based nursing practice guideline for cold application)이란 대류, 복사, 전도 등의 방식으로 표면에서 비침습적으로 냉을 제공하는 간호중재에 대한 실무 지침을 말한다.
본 연구는 근거기반 실무지침을 개발하는 방법론적 연구이다.
지침의 개발은 Scottish Intercollegiate Guideline Network(SIGN)의 근거중심 가이드라인 개발과정[17]과 병원간호사회 실무지침 수용개작 방법[18]을 토대로 연구목적에 맞게 아래와 같이 기획(1~4단계)과 지침개발(5~8단계), 지침의 평가(9~10단계) 등 총 10단계로 진행하였다.
근거기반실무 활동, 교육 및 연구 경험이 있는 방법론 전문가, 임상실무전문가, 검색전문가로 구성하였다.
국내외 임상실무지침 개발 조직에서 운영하는 웹사이트에서 냉 요법 관련 실무지침의 존재하거나 개발 중인지를 확인하였다.
개발팀 구성원 전원은 병원간호사회에서 제시한 이해관계 선언문[18]을 바탕으로 이해관계 선언하였다.
냉 요법 실무지침의 범위를 한국보건의료연구원(National Evidence-based Healthcare Collaborating Agency, NECA)의 임상진료지침 개발 매뉴얼[19]의 기준에 따라 결정하였다. 지침의 목적은 간호사가 냉 요법을 시행함에 있어 이론적 근거에 기반한 실무를 수행하도록 함으로써 간호중재로서의 냉 요법이 올바르게 적용될 수 있도록 하는데 있으며, 지침의 대상집단은 의료기관에 입원 중인 18세 이상의 성인 환자, 중재는 국소적 냉 요법으로, 관련 전문가는 의료기관에 근무하는 간호사로, 결과변수는 통증, 부종, 출혈, 발열 감소로, 적용 기관은 모든 의료기관으로 설정하였다. 검색기간은 2006년 1월 1일부터 2016년 12월 31일로 하였다. 또한, 지침의 구성에 필요한 임상질문을 작성한 후 지침의 개발 계획을 문서화하였다.
지침의 예비문항이 될 임상질문에 따라 근거를 확인하기 위하여 문헌을 검색하였다. 임상질문에 따라 국외 자료는 Cochrane Library, PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Excerpta Medica dataBASE (EMBASE)에서, 국내 자료는 KoreaMed, Korea Education and Research Information Service (KERIS), National Digital Science Library (NDSL)에서 검색하였다. 냉 요법에 대한 문헌 검색 시 포함 기준은 18세 이상 성인에게 발열, 출혈, 부종, 통증 감소를 목적으로 비침습적, 국소적으로 적용하는 경우로 하였으며, 연구유형은 체계적 고찰, 메타 분석, 무작위 대조군 실험연구(Randomized Controlled Trial, RCT)와 비무작위 대조군 실험연구(Non-RCT)를 포함하였고, 최근 10년 이내에 우리말이나 영어로 출판된 문헌을 대상으로 하였다. 제외기준은 연구대상이 18세 미만의 아동, 동물이나 세포인 경우와 한국어나 영어가 아닌 문헌은 제외하였다. 원문 검토 결과 포함기준에 부합하지 않거나 중복 출판된 논문을 제외하고 최종 논문을 선정하였으며 근거 확인 전략에 따라 1차 검색은 검색 전문가가 수행하였고, 이후 문헌의 제외 또는 포함과정은 개발팀이 논의와 합의를 통해 진행하였다.
선정된 문헌은 지침의 예비문항이 될 핵심질문에 대한 답, 즉 근거를 요약하고 근거등급을 결정하였다. 개별 문헌의 비평적 분석을 위해 NECA의 권장[19]에 따라 체계적 문헌고찰은 Assessment of Multiple SysTemAtic Reviews (AMSTAR), RCT는 코크란의 Risk Of Bias (ROB) 도구, non-RCT는 Risk of Bias Assessment tool for Non-randomized Study (RoBANS)를 이용하여 평가하였다. 각 문헌의 질 평가는 지침 개발팀이 수행하였고 의견이 다른 경우는 만장일치가 될 때까지 합의하는 과정을 통하여 결정하였다.
요약된 근거를 권고 사항으로 기술하고 권고안의 근거수준과 권고강도는 미국 감염병 학회(Infectious Disease Society of America, IDSA)의 근거등급 평가체계[20]를 이용하였다. 근거수준은 I부터 III까지로 평가된다. 등급 I은 하나 이상의 잘 수행된 RCT로부터의 근거를 나타내고, II는 하나 이상의 잘 설계된 non-RCT나 코호트 연구, 환자-대조군 연구로부터의 근거를 의미하며, III는 임상의 권위 있는 전문가 의견이나 조사연구, 전문위원회 보고로부터의 근거를 말한다. 권고강도는 A부터 C까지로 권고강도 A는 권고안 사용을 지지하는 근거가 확실함, B는 보통임, C는 미흡함을 뜻한다. 권고의 강도는 개발팀 4인이 합의 과정을 통해 결정하였다.
근거기반실무 활동, 교육 및 연구 경험이 있는 방법론 전문가, 임상실무전문가로 구성된 지침개발팀은 7단계에서 형성한 권고안 권고 강도에 따라 핵심질문을 토대로 지침의 예비문항을 구성하고 지침의 초안을 작성하였다.
냉 요법 실무와 관련된 교육을 담당하는 자를 외부전문가 집단에 포함하였다. 각 외부전문가는 개인을 식별하지 않도록 무기명으로 자료를 수집함으로써 개발팀과는 독립적으로 평가가 이루어지도록 하였다. 전문가 그룹으로부터 지침의 내용타당도(Content Validity Index, CVI)를 산출하고, CVI 가 0.8 이상인 경우만 지침으로 포함하였다.
전문가 집단의 평가 결과를 토대로 최종 실무권고안을 작성하고 간호지침을 확정하였다.
본 연구는 국립순천대학교 생명윤리심의위원회(Institutional Review Board, IRB No. 1040173-201709-HR-025-02)의 승인을 받아서 진행하였으며, 지침 평가를 위한 전문가 참여자로부터 서면동의를 받은 후 자료를 수집하였다.
근거기반 냉 요법 간호실무지침 개발을 위한 단계별 결과는 아래와 같다.
개발팀은 근거기반실무 활동 및 연구 경험이 있는 방법론 전문가 3인과 근거기반실무 교육 및 활동 경험이 있는 석사학위의 임상실무전문가 1인, 검색전문가 1인을 포함하여 총 5인으로 구성하였다.
국내외 임상실무지침 개발 조직에서 운영하는 웹사이트에서 냉 요법 관련 실무지침이 존재하거나 개발 중인지를 확인하였으나 냉 요법과 관련된 지침을 찾을 수는 없었다.
개발팀 구성원 전원은 이해관계 선언을 완료하고 서명하였다.
지침의 구성에 필요한 임상질문은 9개 영역 총 20개 질문으로 냉 요법 적용 전 사정에 대한 1개 질문, 적응 및 금기 대한 5개 질문, 기구에 대한 2개 질문, 적용 부위에 대한 1개 질문, 준비에 대한 3개 질문, 적용시간 및 회복시간에 대한 2개 질문, 냉 요법 제거에 대한 1개 질문, 부작용 사정과 관리에 대한 2개 질문, 교육에 대한 1개 질문, 기록에 대한 2개 질문으로 Table 1과 같이 작성한 후 지침의 개발 계획을 문서화하였다.

연구방법에 따라 검색한 결과 6,426편의 문헌이 확인되었고, 그중 179편의 문헌을 원문 검토하였다(Figure 1). 원문 검토 결과 포함기준에 부합하지 않거나 중복 출판된 논문을 제외하고 최종 147편을 선정하였고, Appendix 1에 제시하였다. 최종 선정된 논문은 체계적 문헌고찰 및 메타분석 연구 24편, RCT 연구 95편, Non-RCT 연구 28편이었다.

선정된 문헌은 지침의 예비문항이 될 핵심질문에 대한 답, 즉 근거를 요약하고 근거등급을 결정하였으며 임상질문 별 근거요약은 Table 2와 같다. 냉 요법 적용 전 사정에 대한 근거를 제시한 연구는 2편(1.4%)이 있었고 발열 감소를 위한 냉 요법 적용에 대한 근거를 제시한 연구는 4편(2.7%), 통증감소를 위한 냉 요법 적용에 대한 근거를 제시한 연구는 107편(72.8%), 냉 요법 적용기구, 냉 요법 적용 부위, 냉 요법 적용온도에 대한 근거를 제시한 연구는 각각 68편(46.3%), 냉 요법 적용시간, 적용간격에 대한 근거를 제시한 연구는 각각 49편(33.3%), 교육, 기록, 부작용 보고에 대한 근거를 제시한 연구는 각각 1편(0.7%) 등이었다. 20개 핵심질문 따른 관련된 근거에 대한 자세한 내용은 아래와 같다.

냉 요법은 첫째, 통증을 조절하기 위하여 주로 사용되었다. 통증감소와 관련한 107편의 연구 중 65편(44.2%)의 RCT 문헌에서 심장수술[3], 복부수술[4], 분만여성[5], 흉관 제거[21], 동정맥루 천자[22], 보툴리늄 주사[23] 후에 냉 요법을 적용한 경우였으며 통증감소에 효과가 있음을 제시하였다. 이들 연구에서 가장 많이 사용된 냉 요법 기구는 얼음 팩이었고 그 다음은 냉각 젤 팩, 냉각수 순환장치, 얼음을 직접 적용하는 방법 등의 순이었다. 얼음 팩은 분만여성[5], 동정맥루 천자[22], 비수술적 근골격계 질환 환자[24] 등에서 사용되었다. 냉각 젤 팩은 심장수술 후[3], 흉관 제거 전[21], 보툴리늄 주사[23] 전에 사용되었고, 냉각수 순환장치의 경우도 근골격계 수술[6] 후 지속적 혹은 간헐적 방법으로 사용되었다.
둘째, 냉 요법은 부종 감소를 위하여 사용되었으며 근거 문헌은 모두 RCT로 10편(6.8%)의 문헌에서 안면수술[6], 안과수술[7], 개두술[8] 환자에게 적용하였으며 효과적임을 제시하였다. 부종감소에 주로 사용한 기구는 냉동 젤 팩이었다.
셋째, 냉 요법은 출혈 감소를 위하여 사용되었다. 슬관절 수술[9]과 안과수술[7], 경피적 심장중재술[10] 환자를 대상으로한 10편(6.8%)의 RCT 문헌들은 냉 요법이 출혈 감소에 기여했다고 보고하였다. 이들 연구에서는 냉동 젤 팩과 냉각수 순환장치를 주로 이용하였다.
넷째, 냉 요법의 체온 감소 효과에 대해서는 국소적 냉 요법이 발열감소에 도움이 된다는 한 편의 체계적 고찰[11] 연구와 한 편의 non-RCT 문헌[12]이 있었고, 반면에 발열감소에 차이가 없다고 한 한 편의 체계적 고찰과 한 편의 RCT연구가 있었다.
냉 요법을 적용할 때 가장 효과적이면서 안전한 온도를 규명하기 위해서, 냉 요법의 효과가 있다고 보고한 RCT 문헌들을 냉 요법 기구에 따라 분석한 결과는 다음과 같다.
첫째, 얼음 팩은 −4~4℃로 준비하고, 적용하는 동안 온도가 유지되도록 한다. 경피적 심장중재술 환자 50명을 대상으로 한 RCT연구 [10]는 −4~4℃의 얼음 팩을 적용했을 때 혈종 감소 효과가 있다고 보고하였다. 얼음 팩을 사용하여 그 효과를 보고한 선행연구의 대부분은 구체적인 온도를 명시하지 않았지만, 온도를 명시한 선행연구를 토대로 얼음 팩의 적정 온도를 제시할 수 있다.
둘째, 냉각 젤 팩은 0~10℃로 준비하고, 적용하는 동안 온도가 유지되도록 한다. 흉관 제거 환자 90명 대상의 RCT연구[21]에서는 4℃의 냉각 젤 팩을 적용한 군이 대조군보다 흉관제거 시 통증이 더 낮았다. 각막절제술 환자 40명을 대상으로 5~8℃의 젤 팩을 적용한 RCT [7]는 냉 요법 적용군에서 수술 후 통증과 부종, 충혈 감소효과를 보고하였다. 이와 같은 근거 등을 토대로 냉각 젤 팩의 적정온도를 0~10℃로 요약하였다.
셋째, 냉각수 순환장치의 냉각수 온도는 10~15℃로 유지한다. 전방 십자인대 재건술 환자 21명을 대상으로 한 RCT [9]에서는 10℃의 냉각수 순환장치를 적용한 군에서 수술 후 통증이 더 낮았고, 90명의 하악관절 수술 환자를 대상으로 한 RCT [6]에서는 15℃의 냉각수 순환장치를 적용했을 때 수술 부위의 부종이 더 낮았다. 이들 근거 등을 토대로 냉각수 순환장치의 냉각수 적정 온도를 10~15℃로 요약할 수 있다.
냉 요법의 적정 적용시간과 회복시간을 확인하기 위해서 냉 요법의 효과를 규명한 선행연구를 적용 부위 또는 적용 대상군에 따라 분류하여 분석하였다. 냉 요법 기구의 특성에 따라 냉이 유지되는 시간 등이 다를 수 있으나, 무엇보다도 적용 부위의 범위와 해부학적 특성, 적용 목적에 따라 냉 요법 적용시간은 큰 편차가 있었다. 본 연구에서는 냉 요법 적용 부위의 침습정도에 따라 침습범위가 작은 경우, 침습범위가 중간인 경우, 침습범위가 큰 경우 등 세 가지로 근거를 요약하였다.
먼저 침습의 범위가 작은 경우는 동정맥루 천자[22], 보툴리늄 주사[23] 등이 해당된다. 60명의 동정맥루 천자 환자를 대상으로 한 RCT [22]는 냉 요법을 2분간 적용하였고, 36명의 보툴리늄 주사 환자를 대상으로 한 RCT [23]는 5분간 냉 요법을 적용하여 통증에 대한 감소 효과를 보고하였다. 이와 같이 침습의 범위가 작은 부위에 냉 요법을 적용한 선행연구에서는 최소 2분부터 최장 5분까지 적용하였고, 모두 부작용 없이 통증 감소효과가 있다고 보고하였다. 이에 기초하여 볼 때, 간헐적 냉 요법을 침습 범위가 작은 부위에 적용할 경우 2분에서 5분이 적절하다고 요약할 수 있다.
둘째, 침습의 범위가 중간인 경우는 흉관삽입[21]과 회음절개[5] 등이 해당된다. 흉관 삽입 환자 90명 대상의 RCT [21], 분만 여성 114명을 대상으로 한 RCT [5] 등은 냉 요법을 20분간 적용하여 그 효과를 보고하였다. 한편 비수술적 상태에서, 침습의 범위가 중간인 경우와 유사한 냉 요법 적용시간을 제시한 경우는 근골격계 문제[24]와 유방울혈[27] 등이 해당되었다. 근골격계 문제로 냉 요법을 적용한 경우는 33명의 골관절염 환자를 대상으로 한 RCT [24]로 냉 요법 적용시간은 각각 20분이었다. 80명의 흉관 제거 전 환자를 대상으로 한 RCT 연구[28]에서는 냉적용시간이 10분었고, 70명의 회음절개술 환자를 대상으로 한 RCT연구[29]에서는 15~20분이었다. 류마티스 질환자에서 냉 요법의 효과를 평가한 체계적 고찰[30]은 10분간의 냉 요법도 통증감소에 도움이 된다고 하였다. 한편 유방울혈 환자 119명 대상의 체계적 고찰[27]에서 냉 요법 적용시간 평균은 20분이었다. 이러한 비수술적 상태에서 냉 요법을 적용하여 효과를 입증한 연구도 근거요약에 포함할 필요가 있다. 이상의 문헌에 기초하여 침습의 범위가 중간 정도인 경우 간헐적 냉 요법의 적정 적용시간은 일 회 10~20분으로 요약할 수 있다.
셋째, 침습의 범위가 큰 경우는 심장수술[3], 개두술[8], 안면수술[6] 등이 해당되며, 냉 요법 적용시간은 최소 15분부터 최장 30분까지였다. 관상동맥우회술 환자 46명 대상의 RCT[3]는 15분 간 적용한 후 심호흡과 기침 시의 통증을 줄여준다고 보고하였다. 97명의 개두술 환자 대상의 RCT [8]에서는 20분간 냉 적용이 안검 부종과 반상출혈 감소 효과가 있다고 보고하였고, 안면 수술 환자 90명 대상의 RCT [6]는 30분간 냉 요법이 부종감소에 도움이 된다고 하였다. 선행연구를 요약하면 주요 수술과 같이 침습의 범위가 큰 경우 국소적 냉 요법의 적정 적용시간은 일 회 20~30분으로 요약할 수 있다.
지침의 내용 중에서 권고의 강도가 A, 즉 ‘권고안 사용을 지지하는 근거가 확실한’ 권고안은 12개(44.4%)였고, 냉 요법의 사용 목적과 냉 요법을 적용하기 위한 기구 선정, 냉 요법 적용방법에 대한 권고안이 근거수준 I 등급에 의해 지지되었다. 권고의 강도가 B, 즉 ‘권고안 사용을 지지하는 근거가 보통 수준인’ 권고안은 간헐적 냉 요법 적용 시 회복시간에 관한 권고 1개로 전체의 3.6%였다. 지침 중 14개(52.0%)는 ‘권고안 사용을 지지하는 근거가 미흡한’ C에 해당하였고, 냉 요법 전 사정, 냉 요법 적용 부위, 냉 요법 준비방법, 냉 요법 중단기준, 부작용 사정과 관리, 대상자 교육, 간호기록에 관련된 내용이었다.
근거기반실무 활동 및 연구 경험이 있는 방법론 전문가 3인과 근거기반실무 교육 및 활동 경험이 있는 석사학위의 임상실무전문가 1인으로 구성된 지침개발팀은 7단계에서 형성한 권고안 권고 강도에 따라 핵심질문을 토대로 지침의 예비문항을 구성하고 지침의 초안을 작성하였다.
외부 전문가 평가를 위하여 냉 요법 실무와 관련된 교육을 담당하는 성인간호학 또는 기본간호학 교수, 실무전문가로서 전문간호사 자격을 갖고 업무를 수행하고 있는 전문간호사, 그리고 지침의 실제적 사용자가 될 임상간호사를 전문가 집단에 포함하였다. 구체적인 구성을 살펴보면 서울, 전남, 충청 지역의 평균 교육경력 13년, 임상경력 5년의 성인간호학 전공 5인 및 평균 교육경력 5년, 임상경력 16년의 기본간호학 전공 5인의 간호대학 교수 10인, 신경외과, 정형외과, 흉부외과, 종양내과 전문간호사 10인, 소화기 내과, 노년내과, 심장내과, 신경외과 임상경력 10년 이상의 간호사 10인 등 총 30인으로 구성된 외부 전문가 그룹에서 평가하였다.
예비 지침에 대한 외부 전문가 평가 결과 CVI는 0.73~1.00이었으며, 그 중 ‘수술상처 등 개방상처 부위에 얼음 팩을 적용할 때는 얼음물이 누출되어 상처가 오염되는 것을 예방하기 위해 플라스틱 필름으로 얼음 팩을 감싼다’는 문항의 CVI가 0.73으로 나타나, 해당 문항을 삭제하였다. 또한 기록과 관련된 권고안 중 표현 상 모호한 부분이 있다는 지적에 따라 ‘냉 요법 중단 및 중재 결과’에 ‘대상자의 반응’을 추가하는 등 일부 문구를 수정하였다.
본 연구에서 개발된 근거기반 냉 요법 간호실무지침은 냉 요법 전 사정, 냉 요법의 목적과 금기, 기구선정, 적용 부위, 준비사항, 적용방법, 냉 요법의 중단, 부작용 사정과 관리, 교육, 기록 등 10개 영역, 27개 권고안으로 구성되어 있으며, 개발된 지침의 주요 내용은 다음과 같다(Table 3). 각 영역별로 권고안은 냉 요법 전 사정 1문항, 냉 요법의 목적과 금기 5문항, 기구 선정 4문항, 적용 부위 1문항, 준비 3문항, 적용방법 6문항, 중단(제거) 1문항, 부작용 사정과 관리 2문항, 대상자 교육 2문항, 기록 2문항으로 제시하였다. 근거기반 냉 요법 간호실무지침에 대한 내용은 Appendix 2와 같다.

본 연구에서는 냉 요법과 관련된 최근 10년간의 선행연구에 대한 체계적이고 비평적 분석과정을 거쳐 10개 영역, 27개 권고안으로 성인 환자를 위한 냉 요법 간호실무지침을 개발하였다. 개발된 지침은 냉 요법 전 사정, 냉 요법의 목적과 금기, 기구선정, 적용 부위, 준비 사항, 적용방법, 냉 요법의 중단, 부작용 사정과 관리, 교육, 기록을 주요 내용으로 포함하였다. 이중 가장 중요한 연구결과로 볼 수 있는 냉 요법의 목적과 기구선정, 냉 요법 기구의 적정온도, 냉 요법의 적정 적용시간 및 회복시간을 중심으로 논의를 하고자 한다.
본 연구에서는 냉 요법의 목적을 규명하기 위하여 주요 결과 변수인 통증, 부종, 출혈, 발열로 구분하여 냉 요법의 효과를 비평적으로 분석하였다. 선행연구를 고찰한 결과 심장수술[3], 복부수술[4], 분만여성[5], 흉관 제거[21], 동정맥루 천자[22], 보툴리늄 주사[23] 후에 적용한 냉 요법이 통증 조절 효과가 있다고 보고하였고 안면수술[6], 안과수술[7], 개두술[8] 환자에게 적용한 냉 요법이 부종감소에 도움이 된다고 보고하였으며 슬관절 수술[9]과 안과수술[7], 경피적 심장중재술[10] 환자를 대상으로 한 RCT 문헌들은 냉 요법이 출혈 감소에 기여했다고 보고하였으나 국소적 냉 요법의 발열감소에 대해서는 발열감소 효과를 보고한 문헌 2편[11, 12]과 발열감소의 효과가 없다고 보고한 문헌 2편[25, 26]으로 냉 요법의 효과를 확인 할 수 없었다. 그러나 우리나라 의료기관에서의 냉 요법 적용 실태연구[14] 결과에서는 발열 환자에게 적용한 경우가 62%로 수술이나 처치 후 통증, 부종, 출혈 감소 목적으로 사용한 경우보다 월등하게 많았다. 따라서 발열감소 효과를 확인하는 잘 계획되고 반복적인 RCT연구를 통하여 그 효과를 확인하고 이를 근거로 임상실무에 적용해야 할 것이다.
문헌고찰 결과 통증감소를 위하여 가장 많이 사용된 냉 요법기구는 얼음 팩이며 냉동 젤 팩, 냉각수 순환장치 등이 사용되었고 부종 감소를 위해서는 냉동 젤 팩을 가장 많이 사용하였으며 출혈 감소를 위해서는 냉동 젤 팩과 냉각수 순환장치가 주로 사용되었다. 그러나 우리나라 의료기관에서의 냉 요법 적용 실태연구[14] 결과에서는 주로 발열감소를 위하여 냉동 젤 팩24%, 얼음 팩 23%, 얼린 수액백 19%, 비닐 수액백 4% 등을 사용하였다고 보고하고 있다. 이러한 결과로 얼음팩과 젤팩은 보편적으로 사용되는 기구임을 알 수 있었고, 특히 냉각수 순환장치는 우리나라에서 보편적으로 사용되는 기구는 아니었지만 통증이나 출혈 감소를 위한 효과가 확인되었으므로 임상에서 적용한다면 환자 안위도모에 많은 도움이 될 것으로 보여진다. 그러나 특정 형태의 냉 요법 기구가 더 효과적이라는 근거는 충분하지 않으므로, 냉 요법의 목적과 대상자의 상태, 선호도를 고려하여 냉 요법 기구를 선정해야 할 것이다.
냉 요법을 적용할 때 가장 효과적이면서 안전한 온도를 규명하기 위해서, 냉 요법의 효과가 있다고 보고한 RCT 문헌들을 각 냉 요법의 기구에 따라 분류하여 분석하였다. 얼음 팩은 −4~4℃로 준비 및 유지할 때 혈종이 감소되었음을 확인할 수 있었고[10], 냉각 젤 팩은 0~10℃로 준비 및 유지할 때 수술 후 통증과 부종, 출혈 감소를 확인할 수 있었으며[7], 냉각수 순환장치의 냉각수 온도는 10~15℃로 유지[6, 9]할 때 효과가 있음을 보고하였다. 본 연구에서 구체적인 온도를 제시하지 않았지만, 온도를 제시한 선행연구를 토대로 적정 온도를 요약하였다. 따라서 이는 참고 수치로서 냉 요법 기구의 크기나 두께, 매체 등의 특성에 따라 달라질 수 있음을 고려해야 할 것이다.
냉 요법의 적정 적용시간과 회복시간을 확인하기 위해서 냉 요법의 효과를 규명한 선행연구를 적용 부위 또는 적용 대상군에 따라 분류하여 분석하였다. 냉 요법 기구의 특성에 따라 냉이 유지되는 시간 등이 다를 수 있으나, 무엇보다도 적용 부위의 범위와 해부학적 특성, 적용목적에 따라 냉 요법 적용시간은 큰 편차가 있었다. 침습의 범위가 작은 경우는 동정맥루 천자[22], 보툴리늄 주사[23] 등이 해당되며 최소 2분부터 최장 5분까지 적용하였고, 모두 부작용 없이 통증 감소효과가 있다고 보고하였다. 침습의 범위가 중간인 경우는 흉관삽입[21]과 회음절개[5] 등이 이며 냉 요법을 20분간 적용하여 그 효과를 보고하였다. 또한 비수술적 상태에서, 침습의 범위가 중간인 경우와 유사한 냉 요법 적용시간을 제시한 경우는 근골격계 문제[24]와 유방울혈[27] 등이 해당되며 간헐적 냉 요법의 적정 적용시간은 일 회 10~20분으로 보고하였다. 침습의 범위가 큰 경우는 심장수술[3], 개두술[8], 안면수술[6] 등이 해당되며, 냉 요법 적용시간은 최소 15분부터 최장 30분까지였다. 이상의 논의에서 본 바와 같이 냉 요법 적용시간은 간헐적 냉 요법의 일회 적용시간이므로 적용 간격 또는 회복시간을 명시할 필요가 있다. 그러나 냉 요법의 효과를 보고한 대다수의 문헌들은 회복시간을 구체적으로 명시하지 않고 있어 이에 대한 근거요약이 제한적으로 이루어졌다. 개두술 환자 대상의 연구[8]는 각각 40분 간격으로 냉 요법을 적용하였고, 안면수술 환자 대상의 연구[23]는 90분 간격으로 냉 요법을 적용하였다. 국소적 냉 요법은 과도한 조직온도 감소로 인한 부작용[15]을 예방하기 위해서 일정한 간격을 두고 냉 요법을 적용함으로써 회복시간을 확보해야 한다. 따라서 이상의 문헌에 기초하여, 간헐적 냉 요법을 적용할 때는 최소 40분 이상의 회복시간을 갖는 것이 필요하다고 요약할 수 있다.
개발된 27개의 지침 중 14개(52.0%) 지침은 ‘권고안 사용을 지지하는 근거가 미흡한’ C에 해당하였다. 이는 질높은 권고안을 개발하기 위하여 냉 요법에 대한 핵심질문에 타당성을 부여할 수 있는 잘 계획된 실험연구가 부족했고 주로 이들의 내용은 전문가 의견이나 조사연구, 전문위원회 보고로부터의 근거가 대부분이었기 때문이었다. 냉 요법의 사용 목적과 냉 요법을 적용하기 위한 기구 선정, 냉 요법 적용방법 등을 위한 지침과 관련해서는 잘 계획된 실험연구가 많았던 반면 냉 요법 전 사정, 냉 요법 적용 부위, 냉 요법 준비방법, 냉 요법 중단 기준, 부작용 사정과 관리, 대상자 교육, 간호기록에 대한 내용에 대한 잘 계획된 실험연구는 아직 부족하므로 질 높은 권고 안을 제시하기 위하여 추후 연구가 시행되어져야 한다.
냉 요법은 통증과 부종, 출혈 감소의 목적으로 사용할 수 있으며, 간헐적 방법과 지속적 방법으로 적용할 수 있다. 냉 요법은 목적과 대상에 따라 얼음 팩, 냉각 젤 팩, 냉각수 순환장치 등을 사용할 수 있으며, 기구의 특성을 고려하여 적정 온도로 적용되어야 하고, 적용 부위에 잘 밀착되도록 고정한다. 또한 안전하고 효과적인 냉 요법을 위해 적용 목적과 대상자 특성을 고려하여 적용 시간을 결정해야 한다.
본 연구에서 개발된 근거기반 냉 요법 간호실무지침은 임상 및 지역사회 간호 실무에 활용될 수 있으며, 냉 요법 관련 간호실무 편차를 개선하는 데 기여할 수 있을 것으로 기대한다.
