The Effects of Anxiety, Depression and Self-Esteem on Smoking and Nicotine Dependency Among Unmarried Woman Smokers

PURPOSE
The purpose of the study was to investigate the effects of anxiety, depression and self-esteem on the amount of smoking and nicotine dependency among woman smokers.
METHOD
The convenience sample consisted of 121 unmarried woman smokers. The STAI, the CES-D Scale, the Self-Esteem scale and the FTQ were utilized to measure the level of anxiety, depression, self-esteem, and nicotine dependency. The amount of smoking was calculated based on the number of cigarette used.
RESULTS
Results indicated that woman smokers were depressed. Nicotine dependency was relatively low. Subjects had smoked a mean of 9.58 cigarettes per day. The level of depression influenced on smoking and nicotine dependency. Self-esteem influenced on the amount of smoking and anxiety influenced on nicotine dependency.
CONCLUSION
The results of this study revealed that the amount of smoking of unmarried woman smokers was very high. Both the amount of smoking and nicotine dependency were affected by levels of depression. Education programs and campaigns are needed to prevent smoking of young adult women.

PURPOSE
This study purposed to examine the disease-related knowledge level and compliance with good health behavior in patients with myocardial infarction according to the atherosclerotic risk factors.
METHOD
The subjects consisted of 72 patients with myocardial infarction and the data were collected by interviewing the subjects with questionnaires and reviewing their medical records from September, 15, 1999 to July 31, 2000. Data were analyzed using the SAS program.
RESULTS
1) With regard to atherosclerotic risk factors: of the subjects, 91.7% lacked regular exercise, followed by smoking (61.1%). 2) The average knowledge score of the patients was 19.7 and the average compliance score was 53.9. 3) There were no significant differences in the total knowledge scores according to the patients' atherosclerotic risk factors. 4) Non-diabetics were significantly higher in knowledge scores on domain of risk factors than the diabetics. 5) The overweight patients were significantly higher in knowledge score on domain of nature of disease than the normal-weight patients. 6) The total compliance scores of the non-smokers were significantly higher than those of the smokers. 7) The total compliance scores of the patients who do regular exercise were significantly higher than those of the patients who forgo regular exercise. 8) The non-smokers were significantly higher in compliance scores on domain of diet than the smokers. 9) The diabetic patients were significantly higher in compliance scores on domain of smoking cessation than the non-diabetics. 10) Patients who do regular exercise were significantly higher in compliance scores on other domains than the patients who forgo regular exercise.
CONCLUSION
According to the above findings, it can be concluded that intensive nursing care and education should be provided to patients who have atherosclerotic risk factors such as smoking, hypertension, diabetes mellitus, lack of exercise, over weight, or hypercholesterolemia to increase disease related knowledge level and to improve compliance with good health behavior.

Meaning of Sickness for the Elderly in a Folk Healing Practicum

PURPOSE
This ethnography was conducted to describe the meaning of illness of the elderly in traditional folk healing performance.
METHOD
This study was guided by Klienman's explanatory model of health care systems. The fieldwork was conducted in an agricultural clan of Namwon City from January of 1990 to Feburary of 2001. Research data were collected by Ehnographic interview and participant observation. Participants of this study were 10 elders aged 74 years old to 96 years old; two of them were male. The data were analysed with the techniques of taxanomy, flow and decision, and proxemics.
RESULT
The meaning of illness was categorized with four compononts, that is, ritual for life, defeat and failure in power game, humiliating punishment for guilt, and Tal. CONCULSION: These meanings were constructed on physical and socio-cultural environment of this clan. The healing strategies were determined based on the meanings of illness. These results can be used to understand the health behavior of the elderly and thus ensure the quality of nursing for the elderly.

Predicting Factors of Breast Self-Examination Among Middle Aged Women

PURPOSE
The purpose of this study is to identify predicting factors of the performance of breast self-examination (BSE) among Korean middle-aged women based upon the Health Belief Model.
METHOD
A descriptive design was used for this study. A total of 309 convenience samples were recruited from Yonsu-Gu, Inchon. The Champion's Health Belief Model Scale was used to measure the health belief related variables of susceptibility, severity, benefits, barriers, confidence, and health motivation. The performance of BSE asked of it was as ever or never performed during the last year. The obtained data were analysed using descriptive statistics, x2-test, t-test, and logistic regression.
RESULT
Results showed that 32% had ever BSE last year. Age and BSE education among demographic characteristics were significantly associated with the performance of BSE. Thus, these demographic variables were added to the logistic regression analyses with the health belief variables. As a result, age, BSE education, health motivation, and confidence significantly explained the performance of BSE.
CONCLUSION
This study suggests that it is important that the development of BSE educational programs increase confidence and motivation, particularly for middle aged-Korean women.

A Study on the Perceived Causal Attribution of Cancer Patients

PURPOSE
The purpose of this study is to identify factors of the causal attribution of cancer and to determine related variables.
METHOD
Subjects were one hundred and thirty three cancer patients. The tool of the perceived causal attribution used was developed by authors and basically founded on Kim's work(1993). The SAS program was used to analyze the data along with descriptive statistics, t-test, ANOVA, Duncan's Multiple range test, and Principal component analysis and varimax rotation.
RESULTS
1) The perceived causal attribution measurement revealed four factors; overload, destiny, stress, and constitution. The total percentage of variance explained by the four factors was 44.3%. 2) The scores of destiny on women, having religion, unemployed, lower level of education, no spouse, groups of uterine cervix and lung cancer, not receiving an operation and receiving radiation were significantly higher than those other groups. 3) The scores of stress on women, having religion, and not having a job were significantly higher than those on men, without religion, and having a job. 4) The scores of constitution on those in their forties, women, not receiving an operation and receiving radiation were significantly higher than for those in their sixties, men, receiving operation and not receiving radiation. There was no significant difference in the factor scores of overload by any variables.
CONCLUSION
Factors of the perceived causal attribution of cancer among Korean cancer patients were overload, destiny, stress, and constitution. The scores of each factor the perceived causal attribution was significantly different by general and disease related characteristics.

An Analysis on the Contents and the Trend of Nursing Research Published in Journal of Korean Academy of Adult Nursing (1989-2000)

"Journal of Korean Academy of Adult Nursing" has been published since 1989 and more than 330 articles have been published up 2000. We reviewed all the articles published in this journal to analyze the contents and trend of published articles and to find out the way of improvement both in quantity and quality of studies in this area. The results are as follows; 1) Ninety percent of articles were quantitative studies. Experimental design was used in 16.1% and quasi-experimental design was adapted in 73.6% of articles. Qualitative research were 10.3% of total articles reviewed. 2) Only 5.1% of articles described theoretical framework and 73.6% of experimentally designed articles included research hypotheses. 3) Among experimental studies, only one was true experimental studie design. Eighty nine point three persent had descriptive survey method among non-experimental studies. Amog the thirty four qualitative studies, phenomenologic(73.5%), grounded theory(14.7%), and ethnographic (11.8%) design were used. 4) Most of research subject were patients and few healthy people, healthcare worker, and patient family. 5) Psychosocial data collection method which used questionnaire were 53.7% and interview, physical measurement, observation were used in 22.8%, 8.2%, 5.4% of articles respectively. 6) Total number of keywords were 692, and average number of key words per article was 2.2. Health-related keyword was the most frequently used according to four metaparadigm of nursing (human, health, nursing, and environment). Environment related keyword was rarely appeared. The number and quality of nursing research in "Journal of Korean Academy of Adult Nursing" have been improved continuously. It is difficult to find out special trend or characteristics which is unique in this journal because of relatively short history of adult nursing studies. This review study was performed to analyze adult nursing studies only in the view of quantitative aspect, but it is needed to analyse qualitatively to find philosophical or theoretical trend in nursing.

The Effect of Music Therapy on Cognitive Function, Behavior and Emotion of Dementia Elderly

PURPOSE
This study was to observe the effects of music therapy on the cognitive function, behavior, and emotions of elderly dementia patients, and to seek musical mediation for them. This study was conducted with patients in the Dementia Sanitarium in C City from March 13 to April 17.
METHOD
The design of research was a nonequivalent control group non-synchronized design and the subjects were 25 patients-15 of whom were in the experimental group with 10 in the control group. The music therapy consisted of favorite music listening in the morning, favorite music group singing activity after lunch, and relaxing music listening after dinner. The schedule was followed 6 days a week for 2 weeks for a total of thirty-six session. The effect of music therapy was measured by MMSE-K and the behavior and emotion measuring equipment which had been derived by the researcher. The verification of the effects is that the score of cognitive function, behavior, and emotions of the experimental and the control group which were measured after the therapy had been applied was analyzed by descriptive statistics and t-test using SPSS WIN program.
RESULT
1)The degree of cognitive function of the experimental group which was received the music therapy is 11.53+/-5.37 which is a little higher than the control group which is 11.20+/-6.32, but it is not significant statistically (t= .14, p= .887). The first hypothesis which had assumed the recepients would have had a higher cognitive function level than the other was rejected. 2) Behavior score of the experimental group that received the music therapy is 68.90+/-7.86 which is higher than the control group which is 66.40+/-11.13, but it is not significant statistically(t= .61, p= .548). The second hypothesis which had assumed the recepients would have had a higher behavior level than the other was rejected. 3)Emotions score of the experimental group that received the music therapy is 42.13+/-5.04 which is higher than the control group which is 35.20+/-6.12, and it is significant statistically(t=3..09, p= .009). The third hypothesis which assumed the recepients would have had a higher emotion level was supported.
CONCLUSION
music therapy which is composed of listening to music and group singing activity is an effective strategy for improvement of the emotions of the dementia elderly. But, the effect of music therapy on the cognitive function and behavior of elderly dementia patients is not significant statistically.

Perceived Powerlessness in Hospitalized Elderly Patients

PURPOSE
The purpose of this study was to identify the degree of perceived powerlessness in hospitalized elderly patients of the medical and surgical unit.
METHOD
The instrument for this study was the Powerlessness Behavioral Assessment Tool(Miller, 1983). The reliability of the instruments was .85. The data was analyzed by descriptive statistics, t-test, and ANOVA.
RESULTS
The results were as follows; 1. The mean score of powerlessness was 35.02(SD=+/-9.24) in a range of 19 to 64, and the mean of powerlessness was total 1.84. Among 4 subscales, the highest score was in the area of verbal response (M=1.98), and the lowest score for powerlessness was in the area of daily activities(M=1.74). In all items, 'verbal expression of fatalism' showed the highest score(M=2.78), and 'verbal expressions of giving up' showed the lowest score(M=1.38). 2. There were significant differences in the level of powerlessness according to hospitalization experience(t=-3.03, p=0.006), medical treatment experience(t=.291, p=.004). Especially, there was significant difference according to the hospitalization experience of the hospital in all sub-scales.
CONCLUSION
Based on these conclusions, nursing education for patient's diseases and treatment can be used for proper nursing intervention in reducing the level of powerlessness of hospitalized elderly.

Predicting Exercise Behavior in Middle-aged Women: Extended Theory of Planned Behavior

PURPOSE
The purpose of this study was to verify the effectiveness of the model based on the TPB, behavior-related theories, and exercise-related empirical studies in predicting exercise intention and behavior.
METHODS
The subjects who participated in this study were 152 middle-aged women. The data were analyzed by use of SAS PC program and LISREL 8.12a program.
RESULTS
1) The overall fit of the hypothetical model to the data was acceptable(x2 24.01(p=0.0043), x2/df 2.67, RMSEA 0.11, standardized RMR 0.04, GFI 0.97, AGFI 0.84, NFI 0.95, NNFI 0.85). 2) Affect, subjective norm, perceived behavioral control and habit were significant determinants of the exercise intention. Attitude and facilitating conditions were not significant determinants. These six variables explained 43% of the total variance of the exercise intention. 3) Perceived behavioral control and habit were significant determinants of the exercise behavior. Intention and facilitating conditions were not significant determinants. These four variables explained 95% of the total variance of the exercise behavior.
CONCLUSION
This study shows the model's applicabiltiy in explaining exercise behavior of middle-aged women, and suggests that we should focus on perceived behavioral control and habit rather than intention to improve exercise behavior of middle-aged women.

Study on the Change of Fatigue in Gastrointestinal Cancer Patients With the Time Relapse After Chemotherapy

PURPOSE
Fatigue is one of the most common complaints of cancer patients. In this study, we analyzed the change of fatigue level and general symptoms as time go by, so that, we could explain more on the mechanism and change of fatigue in relation with treatment, and explore the influencing factors.
METHOD
The subjects of this study were 50 GI cancer patients who have visited the cancer center of A hospital in Suwon. We measured fatigue by using the Revised Piper Fatigue Scale(RPFS) at the time of starting and finishing induction chemotherapy, and starting the 2nd cycle of chemotherapy.
RESULTS
1) The fatigue score was 2.81, 3.73, and 3.82 in a 10 point scale at the time of starting and finishing induction chemotherapy, and starting the 2nd chemotherapy, respectively. This means fatigue persisted until after the treatment. 2) Fifty two percent of participants complained of some kinds of symptoms when starting the treatment, and the proportion increased up to 92% when finishing the treatment. 3) Fatigue scores were significantly high in patients with fatigue-related symptoms than for patients without those symptoms. 4) Fatigue scores showed significant differences according to patients' general characteristics such as age, educational level, economic status, occupation, diagnosis, hematocrit, weight, and amount of sleepy.
CONCLUSION
We have to develop intervention strategies to reduce fatigue in cancer patients in the consideration of influencing factors.

A Study on the Effect of Supportive Nursing Care to Anxiety Reduction of the Patient Immediately Before Surgery in the Operating Room

PURPOSE
This study was to examine the effects of supportive nursing care on surgical patients in the operating room.
METHOD
This study was nonequivalent control group none-synchronized design. The data were collected during the period of July 26 through October 8, 1999 at K hospital in Kwangju. The subjects for the study were selected from those patients who were admitted to K hospital for abdominal surgery. Forty-eight adult subjects were selected and evenly divided into two groups: 24 for the experimental group and the other 24 for the control group, respectively. The tool used for measuring state and trait anxiety was the one originally worked out by Spilberger(1976), and translated by Kim Jung-Taek and Shin Dong-Kyun(1978) for suitable application for Koreans. Data were analyzed by x2-test, t-test, Chronbach's alpha coefficient using SAS/PC+.
RESULT
Results were as follows: 1. The degree of State anxiety was found to be lower in the experimental group who received supportive nursing care in the operating room immediately before an operation than the control group who did not."(p<.001). 2.There was no significant difference on the post-test blood pressure and pulse rate immediately before operation between the two groups(p>.05).
CONCLUSION
Results of the above study reveals that the supportive nursing care in the operating room is effective in alleviating the anxiety of patients. I suggest that it is highly desirable to apply supportive nursing care for those patients who are waiting for immediate surgery.
